The Manager of Care Coordination has primary responsibility for overseeing daily operations of Care Coordination and supervises staff. Provides for an efficient and coordinated approach to the care coordination program.
REQUIRED:
1. Graduate of an accredited school of nursing, OR; Masters of Social Work (MSW) OR; B.S. degree in related field with strong clinical background.
2. At least four (4) years previous management experience.
3. Five (5) years previous experience in Care Coordination/Social Work, utilization review, or social services.
4. Ability to interact with patient, families, physicians and hospital staff (including coworker/peers). 5. Excellent oral and written communications skills.
6. Experience in CQI concepts and tools and techniques.
PREFERRED:
1. Seven (7) years experience in management, social services or related field.
2. Certified as Certified Case Manager.
