Responsibilities

The NHHI Program coordinator will work collaboratively, with the Director of Program Development and Quality, the Medical Director(s) and Hospital/Practice leadership to develop and coordinate program activities in order to achieve and maintain the highest clinical outcomes and a superior patient experience in our Heart Failure program. The NHHI Program coordinator designs, implements, and evaluates system-wide program components, as applicable, for each clinical entity within the scope of the assigned NHHI program. This person is responsible for development, maintenance, and implementation activities relating to growing the program, quality improvement, data management, outreach and marketing, utilization of support services, capital equipment and supply selection, etc.

The NHHI Program Coordinator has primary oversight of physician practice coordinators for the heart failure clinics. This person is also responsible for the heart failure nurse navigation program. The NHHI Program Coordinator also has responsibility for the oversight of the heart failure clinics including support staff, physicians, APPs and daily operations.

 

Qualifications

KNOWLEDGE SKILLS AND ABILITIES/LICENSE OR CERTIFICATION REQUIRED

 

1. Registered nurse

2. Minimum 5 years’ experience with Cardiology, Heart failure programs/patient population

3. Strong administrative, management, and supervisory experience as evidenced by the       candidates ability to:

    1. Plan, organize, direct, and control activities of an organization, department, or service within or across a hospital or similar entity
    2. Establish and maintain effective working relationships with staff, management, providers, and others allied health professionals
    3. Ability to educate others and serve as a positive change agent.
