Responsibilities

Provides clinical pharmacy services including but not limited to assessment, monitoring and optimization of patient specific pharmaceutical care. Serves as a departmental and organizational resource in antimicrobial stewardship and provides educational programming and staff development to physicians, pharmacists, nurses and other practitioners in infectious diseases. Serves all patient populations including neonates, pediatrics, adolescents, adults and geriatrics.

 

Antimicrobial Stewardship Clinical Pharmacist

  1. Performs duties of a clinical pharmacist in providing pharmaceutical care to patients receiving antimicrobial therapy.
  2. Monitors all aspects of medication use related to patients being treated for infectious diseases.
  3. Develops and maintains effective working relationships with key physicians and nursing personnel in service area.
  4. Participates in quality initiatives including evaluation and analysis of medication use practices.
  5. Assists in development of medication use protocols specific to area of practice.

Qualifications

REQUIRED

  1. Graduate of an accredited School of Pharmacy.
  2. Doctor of Pharmacy, OR  Master's Degree with completion of a residency in the assigned clinical area or equivalent experience.
  3. Two (2) years previous hospital pharmacy experience.
  4. Ambulatory Care Clinical Pharmacist – One (1) year previous hospital experience and two (2) years previous outpatient experience.

PREFERRED

  1. Infectious Disease Pharmacy Specialty Certification (BCIDP) by Board of Pharmacy Specialties (BPS) or Society of Infectious Diseases Pharmacists’ Antimicrobial Stewardship Certificate Program is strongly encouraged
